Do you bring a frequent guest and would prefer to not have to fill out the guest waiver form every time?
This new form will be good for the calendar year and will make it
so there is no need to fill out a form every time you bring a frequent guest.
To get a form, please e-mail the club with [Waiver Form] in the subject line.
Remember to include your name and address!
We will mail the form to you, then you will fill the form out and return it to us with a check for $20.00.
You will receive a frequent guest waiver permit number.
Use the permit number on the m/g line of the sign-in sheet when you and your guest sign in.
